What “anti-aging” should mean in 2025
In clinical practice, “anti-aging” means adding healthy, disability-free years (healthspan). GLP-1s help by reducing risks that shorten quality of life—heart attack, stroke, kidney failure, and progressive liver fibrosis—rather than promising to “turn back the clock.”

What’s not proven: “Biological age” reversal
Epigenetic “clocks” (DNA-methylation scores) are promising research tools, but they’re not validated clinical endpoints. Translation: great for labs and future trials—not for promises to patients today. Our job is to lower hard outcomes you can feel: fewer heart/kidney/liver complications and more strong, active years.

Who’s a good candidate—and who isn’t
Often good candidates
· CVD + overweight/obesity → consider Wegovy® for CV risk reduction (case-by-case eligibility).
· Type 2 diabetes + CKD → consider Ozempic® for CKD/CV-death risk reduction.
· MASH with moderate–advanced fibrosis (no cirrhosis) → consider Wegovy® (accelerated approval).
Not appropriate / use caution
· Personal/family history of medullary thyroid carcinoma or MEN2.
· Discuss risks: GI side effects, gallbladder disease, pancreatitis, diabetic retinopathy progression, and peri-anesthesia aspiration considerations.
· Shared decision-making is essential.
How to “age well” on a GLP-1 (our clinic protocol)
· Protein target: ~1.0–1.2 g/kg/day (adjust for kidney status).
· Resistance training: 2–3 sessions/week to protect against sarcopenia during weight loss.
· Monitoring: BP, A1c (as relevant), lipids; consider DEXA if high sarcopenia risk; routine hepatic/renal labs per indication.

Is Ozempic an “anti-aging” drug?
Not as an approved claim. Ozempic® is approved for type 2 diabetes and for reducing the risk of CKD progression/kidney failure/CV death in adults with T2D+CKD. Wegovy® is approved to reduce CV death/heart attack/stroke in adults with CVD and overweight/obesity. Neither is approved to “reverse biological age.”
Can GLP-1s make my epigenetic age younger?
There’s no clinical consensus. Epigenetic clocks are exploratory and not validated for individual treatment decisions or marketing claims.
What about liver benefits?
Wegovy® has accelerated approval for non-cirrhotic MASH with moderate–advanced fibrosis. Lifestyle therapy remains foundational; confirmatory data are pending.
Do older adults tolerate GLP-1s?
Many do, but expect GI side effects during titration. Screen for sarcopenia, set a protein/strength plan, and review label precautions (e.g., thyroid tumor warning, pancreatitis).
I don’t have diabetes. Could a GLP-1 still help me live longer?
If you have established cardiovascular disease plus overweight/obesity, Wegovy® is indicated to lower CV risk. Otherwise, your clinician will weigh benefits/risks and alternatives.
